File of Life
Under the "File of Life"
program, elderly people, or anyone else with a medical problem, can keep
a summary of their medical history on their refrigerator where the EMTs
can get it right away. Each kit includes a magnetic plastic insert and a
medical history form to complete. The form lists the patient's medical
information. You put a copy of the form in the file and place it on the
refrigerator door.

Center to pick up a File of Life if you do not have one or to pick up
extra blank medical forms.
